Carren Stratford, who works at John Peter Smith Hospital, was critically injured in January when she got trapped by an elevator as it went up. She was crush as a result.

What Happened Here

Following the incident that injured their nurse, the hospital hired an outside consultant to survey the work ThyssenKrupp had been doing. The consultant found that TKE was not performing up to a high level when it came to the job they were contracted to do. Debris and oil were found at the bottom of elevator shafts, there were discolored ropes, and some electrical equipment was missing covers.

In fact, the consultant had recommended that the elevator that caused the injuries be updated two years before the incident took place.
